Every patient is unique with many different type of needs.
We first communicate with the patient and family members to discuss likes, dislikes, and the challenges they are facing. Once we have determined the type of care needed, our care management team creates a highly individualized plan of care, which includes matching up the right medical staff for the patient.

Social Services
Medical social workers help patients and their family cope with the personal and emotional impact of their illness or disabilities. The social worker may also assist with locating financial resources and community assistance programs such as IHSS, home meal delivery or support groups.
Home Health Aides
Angelicum home health can provide home health aides to help with the patient's personal care or hygiene needs.
Nutritional Services
Registered dietitian provides evaluation, consultation, and teaching of patient's nutritional needs.
